Health & integrated care
Connecting care, support and service pathways
People can experience gaps when health, social care, housing, education and community services hold different parts of the picture. Ardenvia can support work that requires clearer interfaces, responsibilities and practical pathways.
Relevant contexts
This may include support around hospital discharge, admission prevention, mental health and section 117 pathways, transitions between children’s and adult services, community support, workforce practice, safeguarding and cross-organisational governance.
Our contribution
Ardenvia can help map the current pathway, identify friction and unmet need, clarify roles, review practice and documentation, engage stakeholders and develop a prioritised improvement plan. We will identify where clinical, legal, commissioning or statutory decisions remain with the responsible body.
Working across systems
Cross-system work needs more than a diagram. It requires agreement about thresholds, information sharing, escalation, funding, accountability and what happens when normal arrangements fail. The work should remain grounded in the experience of people using the pathway and staff delivering it.
Start with the pathway question
Describe the population, pathway, partner organisations, current pressure and decision required. We will propose a proportionate first step.
