Adult social care should support people to live with dignity, choice and as much independence as possible while responding proportionately to risk, safeguarding and changing need.

A whole-person view

We consider the person’s goals, communication, relationships, culture, health, environment, strengths and support network. We also consider whether organisational arrangements help staff provide consistent support. Individual plans and service systems should reinforce rather than contradict one another.

  • Coordinate across social care, health, housing and community support.
